Re: Prospectus 5/29/2020 What is the real difference between CRS and cytokine storm? I imagine it is just a case of terminology. Both refer to undesirable systemic inflammatory response. 'Storm' just suggests heightened CRS. Anti-IL6 drugs such as Tocilizumab are generally effective to damp down the dysregulated immune response. But Car-T would be a more attractive treatment if it didn't almost routinely cause CRS, which can range from mild to potentially fatal. ( Though for the life of me, I don't know why Tocilizumab isn't in widespread use for Covid CRS yet...) The neuro-toxicity is at least of equal concern, and it is poorly understood. This is what happens when a new class of drugs is rushed to approval prematurely. And now there are question marks about duration of response, with perhaps half of patients relapsing within a year. This is supposed to be one-time treatment.... Allo Car-T's? Well OK, an anticipated cost reduction, but safety? Who really knows? And efficacy? Who knows? For sure, the interest in further Car-T research is very strong, but to some extent that is due to the failings or limitations of the initially approved Car-T treatments. Did Gilead pay too much? Yes, almost certainly. Of course, NWBO will have to tread the same path on the the CMC / manufacturing front to demonstrate and ensure critical quality attributes are consistently achieved beyond approval. And this will be the bulk of a BLA. Trial efficacy data is just the start... Of course, this is not widely recognised on another message board I could mention. Car-T requires specialised regional treatment centres, and the incurring of large additional costs beyond the price of the drug. This, alongside toxicity and relapse rates, is why it will remain a small niche treatment at least in the near-term. And obviously NWBO has to avoid the same fate. One big difference is that DCVax has a complete absence of any concerning toxicity. In terms of solid cancers, I don't see Car-T therapy getting anywhere in the foreseeable future. |
